Intake FormsThe intake forms listed below are in PDF format. You should be able to "right-click" on the name of the needed form, select "Save Target As...," and then save the form to your computer. After saving, you can then open the form with any PDF Reader software. The free Adobe Reader that you may already have on your computer will allow you to open and print the form if you would like to complete it by hand, but will not allow you to save the file with the information that you type into the form. However, there are other PDF readers that are also free and do allow you to save PDF files along with the information you type into the form -- PDFFiller is web-based and allows you to fill in the form with your computer, without the need to download software. Another popular program is Nitro PDF Reader. With either of these programs, you can save the filled-in form to your computer and, when done, print the form or email the form to our office as an attachment. These programs also have a "typewriter" function that will allow you to type directly into the forms using your computer, and even type extra notes and information onto the forms. If you have any trouble downloading or opening these forms, or if you're not sure which form to use, please call the office at 703-691-1888 and our Client Service Coordinator can assist you.
