The intake forms listed below are in PDF format. You should be able to "right-click" on the name of the needed form, select "Save Target As...," and then save the form to your computer. After saving this form to your local computer, we recommend you complete it with a PDF program that allows you to save files, such as Nitro PDFReader and PDF-XChange Viewer, both of which are  free (the free Adobe Acrobat Reader does not allow you to save the form).  When done, please email the completed form as an attachment to intake@FarrLawFirm.com or print and bring it to your introductory meeting.
